LifeCare pays a significant contribution to help offset the cost of benefits coverage. For those employees who choose to take no benefits at all, the Company will provide a “Cash Out Option” to the employee.

 
       
   

Health Benefits

  • Medical Insurance—Comprehensive coverage which provides in and out of network plan coverage. A PPO and an HSA option are offered. A prescription plan is also included in both plan options through United Healthcare which provides a broad network of pharmacies.
  • Dental Insurance—A dental plan that allows you to visit the dentist of your choice—or receive a discount by selecting a participating network dentist. Orthodontic coverage is also available.
  • Vision—A stand-alone vision care insurance program.
  • Short Term Disability—LifeCare offers a 100% company paid group short-term disability plan providing 60% of your weekly salary up to a maximum benefit of $500.00 per week. A voluntary buy up plan is also available.
  • Long Term Disability—LifeCare offers a 100% company paid long-term disability plan providing 60% of your monthly salary with a maximum benefit of $5,000 per month.
  • Life Insurance/ADD—LifeCare provides a 100% company paid group life insurance plan and accidental death & dismemberment plan covering 1 times your salary up to a maximum benefit of $50,000. LifeCare employees also have the opportunity to purchase additional voluntary coverage of 1, 2 or 3 times their annual salary up to a maximum of an additional $500,000.
  • Long Term Care Insurance—LifeCare provides you with access to a group policy for Long Term Care Insurance available to you and your family members at a discounted rate.

Note—Eligibility for all health benefits is the first of the month coinciding with or following date of hire.

 
       
   

Financial Benefits

  • 401(k) Plan—You are eligible to participate the 1st of the month following your date of hire. LifeCare provides a company match of 100% up to 3%. In addition the Company will match 50% of the next 2% that you elect to defer. Employees are eligible for the Company Match Contributions upon completion of one year of service. Both the Deferred Salary Contribution and the Company Match Contributions (upon eligibility) will be 100% vested and may not be withdrawn while employed prior to the age of 59 ½ .
  • FSA (Flexible Spending Account/Un-Reimbursed Medical)—This program provides tax and income advantages for certain types of health care expenses. Employees are eligible to deposit up to $5,000 into the health care account annually. New hires are eligible for participation on the first day of the month following 30 days of employment.
  • Tuition Assistance Program—LifeCare reimburses 50% tuition for pre-approved course-work, degree programs and certifications (maximum of $5,250 per year).
  • Adoption Assistance—LifeCare reimburses documented adoption expenses up to $5,000 toward the cost of adopting a child.
 
 
       
   

Life Event Services and Benefits

  • Paid Time Off (PTO) Plan
    • 128 hours of paid time off (prorated during first year); 168 hours per calendar year after 2 years; 208 hours per calendar year after 5 years, and each year thereafter.
    • 10 paid holidays (Mix of fixed and personal choice days. Personal choice days are pro-rated based on hire date.)
    • Three paid bereavement days
    • Five paid jury duty days
  • Summer Hours—Every other Friday in the summer, employees enjoy half-days!
  • Flexible Work Arrangements—LifeCare offers compressed work schedules, FlexPlace and FlexTime arrangements.
  • Phased Return to Work Policy from FMLA—Employees can transition back to work after family leave by gradually phasing into a full time schedule.
  • Mothers at Work® Program—Our corporate lactation program that provides lactation consultation, on-site mother's room and a Medela Pump N' Style breast pump paid for by the company.
  • LifeCare ServicesLifeCare's life event services help employees better manage their personal and professional responsibilities by providing counseling, information and referrals on child care, adult care, academic resources, health and wellness services, personal services and more.
  • Employee Assistance Program (EAP)—LifeCare offers a confidential Employee Assistance Program to help employees handle a wide variety of issues such as: financial, credit, legal, tax, pre-retirement planning as well as marriage, family and individual counseling.
  • Legal Access Plan (LAP)—LifeCare offers through its business partner extended legal and financial services.
  • Enhanced Backup Care—LifeCare offers back up care for children and adults.
  • Homework Connection—LifeCare offers through its business partner tutoring and homework assistance.
  • Charitable Giving Program—Corporate matching program for charitable donations of money or time by employees. LifeCare will match up to $250.00 per year.

  • Aflac—3 plans are offered as a voluntary benefit to all employees. Various levels of coverage and rates are available.
    • Personal Accident Indemnity Plan (accident-only insurance policy)
    • Voluntary Indemnity Plan (a hospital confinement indemnity policy)
    • Personal Cancer Indemnity Plan (a specified-disease insurance policy)
  • Direct Deposit—LifeCare saves employees time by having your check deposited directly into your account.
  • Credit Union (Mutual Security FCU)—Membership available to all employees offering various services at competitive rates.
  • Bronx Zoo & New York Aquarium Online Discount Ticket Program —As a member of this program, you are now able to buy discounted tickets for the Bronx Zoo and New York Aquarium with the added convenience of printing your admission tickets from your computer and presenting them at the entrance turnstiles bypassing ticket booth line.
  • Auto and Home Insurance—LifeCare offers through Liberty Mutual discounted auto and home insurance for LifeCare employees.
